    Hi Justin, I have a few more questions. Assuming I move forward with the website BEFORE I do a practice management program, I will need the following capabilities: 1. Fillable Patient Registration Form 2. Fillable Referral Form 3. Online Payment - I currently have integrity as my payment software program. From my understanding the web designer will need a form that has certain fields to capture the client name and any other date we would need. Can you confirm you would be able to do this. Can you confirm that is possible? If so, please forward all documents and I will get started immediately.
