    Nanda was under impression that we will create a form for her and connect it to processing company. CC company sent a document on what to do, but we simply will not put the pieces together.

    Cannot copy the form from previous site as there is coding that resides on previous server. Explained to Nanda that we do not create forms based of third party companies. AND we do not create payment portals for ECommerce. She’s asking for a solution and understands that she was not the one that signed the contract with us, so does not know what’s included.

    “Degenerative Disc Disease occurs when spinal discs lose their hydration and therefore their flexibility and elasticity. Degeneration usually progresses over a long period of time and can be seen in almost everyone as they age.     Herniated Disc will occur when the center of the disc bulges through the outer layer of the disc and puts pressure on the spinal nerves causing radiating pain down the leg.

    Radiculopathy is commonly called a “pinched nerve” or “sciatica” and occurs when a nerve in the neck or low back is compressed or irritated where it branches away from the spinal cord. The pinching may cause pain that radiates into the shoulder, arm, hand or leg. This may also cause muscle weakness and numbness.

    Neck or Back Strain is a fairly broad category of soft tissue injuries of the muscles, tendons and/or ligaments that support the spine. About 80% of back and neck pain is muscle-related.

    Sacroiliac Joint Pain is pain in the sacroiliac joint (often called the SI joint) which connects the tail bone to the pelvis. The SI joint is located just below the lower back and is located on both the left and right side of the body. When a SI joint becomes inflamed this can lead to low back pain and leg pain. SI joint pain can also be increased after a lumbar fusion.

    Scoliosis is a curvature of the spine that occurs most often in the early years of life before puberty. Scoliosis can also be caused by muscular dystrophy or cerebral palsy, but the cause of most scoliosis is unknown.

    Spondylolisthesis is a forward or backward slip of a vertebra over another. This can cause compression of the spinal nerves and lead to abnormal motion in the spine resulting in back and/or leg pain.

    Spondylosis is a general term for age-related wear and tear affecting the discs in your spine. As the discs dehydrate signs of arthritis develop, including bone spurs along the edges of the vertebrae. Symptoms related to spondylosis include arm numbness and pain, gait disturbance, bowel or bladder dysfunction, weakness in the arms and legs and loss of dexterity depending on whether the nerves and/or spinal cord are being compressed.

    Stenosis is a narrowing of the spinal canal which typically results from gradual wear and tear of the spine over time. Bulging of the disc along with bone spurs can cause narrowing of the spinal canal which then puts pressure on the spinal nerves. Spinal stenosis can occur in any part of the spine but most commonly in the lumbar spine. Stenosis or narrowing can cause back and leg pain and make it difficult to walk and stand for long periods of time. Stenosis can also cause weakness in your legs.”

An electrical current produced by a radio wave is used to heat a small area of nerve tissue. When the procedure is effective for pain relief the relief can last for 8-12 months. An RFA can be repeated as needed to address pain related to degeneration of the joints from arthritis. ”

    Lumbar Spine Surgeries (surgeries of the lower back)

    Lumbar Spinal Fusion surgery is performed to join or fuse two vertebrae together. The surgery is intended to stop motion in that part of the spine and therefore decrease pain from degenerated joints. Implants and graft material are inserted to enable healing of the fusion to take place. There are several approaches to spinal fusion surgery including lateral (side of the body), anterior (front), and posterior (back).

    Discectomy is a very common procedure performed to relieve the pressure on the nerves and help decrease leg pain. This minimally-invasive procedure is performed through a small incision with the goal being to remove the portion of the disc that is putting pressure on the nerves. The greatest chance of a recurrent disc herniation is within the first 3 weeks of surgery, so it is very important that you follow the post-operative restrictions provided.

    Laminectomy is a type of surgery performed when the spinal canal becomes too narrow and compresses the spinal nerves, causing pain and sometimes other symptoms. This procedure is performed through the back of your spine to remove the lamina (bone) and/or thickened tissue (ligament) which is causing the narrowing the spinal canal, and therefore will relieve the pressure on the nerves, decreasing leg pain. This operation is commonly performed as part of a decompression and fusion operation, but a decompression may also be performed without a fusion depending on your diagnosis.

    Cervical Spine Surgeries (surgeries of the neck)

    Posterior Cervical Spine Fusion surgery is performed to join or fuse two vertebrae together. This is done through a midline approach through the back of your neck. A decompression is often performed at the same time to relieve pressure on your spinal cord and nerves by removing the bone and ligament that is causing compression. Implants and graft material are then inserted to enable healing of the fusion to take place.

    Anterior Cervical Discectomy and Fusion (ACDF) is performed to address nerve pain caused by disc herniation, spinal stenosis and/or spondylosis. Going through the front of the neck, under general anesthesia, a discectomy will be performed which removes the disc, giving access to the spinal canal. The benefit of the anterior approach is that the disc can be accessed and removed without manipulation of the spinal cord. Bone graft or another material discussed by your surgeon will be used to replace the disc that was removed. Once in place, a titanium plate will be used to hold the graft in place and stabilize the construct.

    Total Cervical Disc Arthroplasty is an alternative to fusion of the vertebrae and, unlike fusions, does preserve motion of the spine. This surgical procedure can be performed in conjunction with an anterior cervical discectomy and decompression. While under general anesthesia, a small incision is made on the front of the neck and an artificial disc is implanted which will maintain motion similar to the native disc. This surgery is designed to address a single level disc herniation or disc degeneration. Neuromonitoring will be used to monitor the function of your spinal cord and nerves.

    Scoliosis Treatment in most cases, the mainstay is observation. In children, depending on growth and severity of curve a brace might be recommended to keep curve from worsening. In severe cases of scoliosis surgery may be recommended.”

    In 2008 Spine Colorado developed a successful research department which has authored and contributed to numerous publications, successfully managed over twenty clinical trials, and implemented various in-house studies.     Surgeons of Spine Colorado, PC provide consulting and research for the development of spine implants and improvement of surgical instruments and techniques, and provide instructional lectures on spine implants, techniques, guidelines for care, and patient outcomes. All of the physicians at Spine Colorado are actively engaged in the study of activities and treatments that may result in better pain relief and a return to a satisfying daily life.

    “Dr. Jim Youssef joined Durango Orthopedics in June 1997, and founded Spine Colorado.

    Dr. Youssef is a board certified, fellowship-trained spine surgeon, the highest level of training for a spine surgeon in the United States. He specializes in minimally invasive spine surgery, fracture care, complex spinal disorders and alternatives to bony fusion in spine surgery.

    Dr. Youssef completed his undergraduate degree in Genetics at the University of California at Berkeley in 1986, and earned his medical degree in 1991 at the University of California, Irvine School of Medicine. He completed his internship in general surgery at Oregon Health Sciences University in 1992 and his residency in orthopedic surgery at Dartmouth-Hitchcock Medical Center in 1996. His did his fellowship in spine surgery at the University of California, Davis Medical Center in 1997.

    In addition to his busy clinical practice, Dr. Youssef oversees an active research department. He has participated in multiple IDE/FDA clinical trials and has published his research in over 25 peer-reviewed publications. Through his research activity, Dr. Youssef is involved in the newest surgical technologies, allowing him to offer his patients the most cutting edge treatments available. Dr. Youssef is a member of the Cervical Spine Research Society, American Orthopaedic Association, North American Spine Society, Lumbar Spine Research Society, Society of Lateral Access Surgery (President), Society for Minimally Invasive Spine Surgery (Founding Member) and other national spine and orthopaedic societies.

    Dr. Youssef is an editor for several peer-reviewed journals, and is the author of several book chapters published in medical textbooks. He is co-founder of the Evidence & Technology Spine Summit, a Continuing Medical Education conference attended by spine surgeons from around the world.     The group met today and the overall consensus was that no one liked the spine image in the S, they felt like it looked like a seahorse. Below are the edits we’d like to see:

    · Top left: try it with a bigger S, and with the colors reversed (mountains purple, text green)
    · Top right: eliminate it
    · Middle left: eliminate it
    · Middle right: change “seahorse” to “S”
    · Bottom left: change “seahorse” to “S”
    · Bottom right: change “seahorse” to “S”; try it with a little bigger S (or bold S)

    Also a concern is that Spine is such a small word compared to Colorado, that a lot of people felt like all they were reading was “PINE” or “S PINE”. Do you have any suggestions on how the spacing could be done different or maybe font would change that? What would you recommend.”

    Adele sent me over some notes regarding my design follow-up. She will be having a meeting with her team to discuss the design and their feedback. Will f/u on Thursday to get complete set of notes.

    1. It is not necessary to have both these sections on the home page. The image of the hikers is a slideshow, something that can’t be seen because this is a static image of the home page design. “Welcome to Spine Colorado” is a short introduction of the office and the top services offered by your practice. It is not necessary to have on the homepage design, especially if you would like a shorter page. Let me know if you would like to keep this area on.

    -Both Kim and I found these two photo sections a bit redundant, perhaps it will look different though with a moving slideshow. Seems to me that most pages have one of these types of sections but not two. Doesn’t seem to add a lot of value. I don’t want to decide on my own, though, and do not have time to get input. Let’s keep this question on hold for now.

    2. We can certainly replace the images with folks who are older. I can update the images on the next draft.

    -Isn’t there going to be another way for patients to be directed to the patient portal, which is how they would request appointments or contact us? I can’t remember but thought we were going to have this prominent at the very top of the page or somewhere else prominent. That is what I prefer rather than a middle-of-the-page section.

    6. The FAQ can be non-clinical, we just need to know what the frequently asked questions should be. Otherwise, we can keep it the same and replace the text at a later time.

    -Sounds good.

    Please let me know if there are changes to colors, layout, and overall design so that I may start working on the new draft.

    -Sorry, but I need input and we are too short-staffed to get it. I’ll try to set up a short meeting next week so that I can get some input from others.

    Jessica, We had a good discussion about the draft layout, and have the following requests for changes: · Please try moving our logo more to the left · We need the option to Pay Bill at the top right of the page. Patient access to our patient portal to schedule appointments and ask questions is extremely important. Please also add this to the top right of the page. So this section would have 1) Pay bill 2) Patient Portal to make an appointment or ask questions · For the page links in purple at the top of the page let’s put: About Us; Physicians & PA’s; Locations; Spine Problems; Treatments; Educational Resources. Do you think we need “contact us” if we put it on the top of the page? · The slideshow format is fine; we will work with you on content for these slides. Instead of “make an appointment” in this slideshow we would like to either eliminate a link or perhaps put a link to our portal. (We had a difference of opinion.) Sorry, but I am on the fence about this. · In the grey banner immediately below the slideshow we would like words only (no shaded pictures) with the links to these pages. We think the page is too busy. · In the Welcome to Spine Colorado section we want to remove the links to spine conditions (we will use them lower) and instead have a link to Patient Testimonials. We will use a patient photo for this section. · In general we like the section on Where is Your Pain?. The figure needs to be facing us, and the areas that will be able to be selected should be neck pain, middle back pain, lower back pain, and leg and/or arm pain. When the person hovers over this section of the front-facing skeleton we’d like the circle with the same name to turn a color, and when click this would activate the link. · Add a section below (or with) the Where is your Pain section that “lists” conditions and allows clicks to links for each. This is where arthritis, disc problems, spinal deformity, pediatric scoliosis, etc would be. · Eliminate Request a Free Consultation. Instead we will have more FAQ’s in this section and they will be non-clinical (e.g., where are your clinics located?; what do I need to bring to my first visit?; what should I expect on my first visit?: do you take my insurance…) The FAQ’s can be across the page rather than only on the right. Could we see a few different tones of color? We’d like to see the purple with a little less red in it. Some like the green; others want to see it with more of a sage green. We are envisioning a bit of an algorithm for patients who click on a condition or pain link. Once they get to that page which describes the condition we’d give an option to request an appointment on the portal, and we might then have the answer to what to expect on my first visit.     Here are some initial thoughts:

    1. I am not sure that the photo of hikers and the Welcome to Spine Colorado sections offer enough value on their own to have both sections. What do you think?
    2. We will want photos of folks who are older—our target population is in the middle to late years.
    3. I like the narrow section “non-surgical…”, “injections trea…”, and “state of the art…” in concept; we may play with the content and words.
    4. I really like the “where is your pain” section!
    5. We don’t offer free consultations; please remove this section.
    6. Kim tells me that the FAQ’s were meant to be non-clinical (about our practice questions) such as where are we located and what insurances do we take, rather than clinical questions. (I missed this part of their meeting.)

    Spine Colorado website redesign:

    · Color Scheme to match / complement our SCO logo colors: purple/violet and light green (but not the sage green currently on our website – we want more vibrant green – sage green is too washed out)

    · Dark font on light background

    · Home page with images that convey we are a nationally-renowned, top quality, highly professional spine center blended with images of active, healthy people, mountain west, scenic landscapes.
    o Incorporate scrolling (horizontally) images / photos that are vibrant, colorful. Possibly combine with scrolling “tag lines” such as, “Rated in the Top 100 spine clinics in the nation.”

    · Home page should be clean (uncluttered) and easy to navigate – limited scrolling down – tabs at the top that show dropdown lists when user hovers over them.

    · Home page featured content:
    o Contact info, especially phone and fax, prominent and easy to see (font size / style on our current website is too faint/light)
    o Our main services that set us apart from the competition – perhaps in a bulleted list with short summaries (we will identify our main services)

    – Kim (Manager) & Adele (CEO) & also 1 of the doctors will be involved
    – Kim is EXTREMELY nice, haven’t talked to Adele  & the doctor yet but Kim said she will be involved in the project especially the interview. She said that Dr is super busy so she will try to find a day/time for the phoen interview. They will be filling out 2-3 questionnaires but understands that it may be different but that will be discussed at the interview. THey just want to get Dr’s input as well but understand that they will get a cohesive agreement in the future. Kim will be the main person that will gather future feedbacks for designer to update the concept.
    – sent questionnaire already, will have that filled out by the time of the interview (will select day/time in the questionnaire)
    – send initial email today, Kim will be on the lookout for it. She has had issues with emails in the past receiving them which she is aware it is on her end so for future communications, try to call if you don’t hear back from her.

    – Its a large orthopedic practice. They have CEO and “board meetings” so they are very professional.
    – looking for a site to rebrand and reflect them better
    – please make sure you connect their G+ listing (i had previously helped her locate it on G and showed her what to do)
    – do not take content from live site cause it is proprietary of the other company. use the library that we have.
    – Kim knows they get 5 custom pages and that future ones will be about $100 per page upon request. Please let me know if you are ordering more
    – have a floating button for Appointment and also for G+ below it. This needs to be prominent and she previously liked it.

    Looking to rebrand the website. Recently had logo done so not looking to change that or any other features. Site is out-dated. Their practice recently separated from a larger group.

    Shopping around right now putting together a spreadsheet for the doctors to compare.

    Went over the Gold which is what she was looking for and need only. Gave pointers on and comparison with us vs other when it comes to quality, pricing, & support.

    Showed her G+ which she is not familiar. gave her tips on what to do and the importance of it. For FB, she s just looking to rebrand it so it is consistent with new design. Optional $20/m on-demand update.

    Asked about guarantee cause others offer this. Offered her 30-day MBG but i did tell her others are prob templates which they can afford to do but when it comes to similar companies like us, no other will do it except us bc we are confident.
