My practice site has been down for a month, the response to my requests has been painfully slow. I simply can’t afford for my practice to be impacted this way. I am cancelling any and all services from Optimized 360 effective immediately. Given the difficulties, no further billing should occur in regards to his matter.

Maps look fine, still not happy with the copy/text in the web site so still not ready for publishing live. We are needing to go through the site and proof read. We need another week or so to go over.

Take screen capture and replace map image in footer. When this image is clicked on, opens an API google map with the 2 locations.

2. Inner pages – center copyright text in footer. Move social icons from header to footer. Make the size smaller so it fits well with footer text.

Dr. Booth will be sending new location to add to site. After final review after this edit, he’s ready to go live.

doctor will be opening up new location in a few weeks. He will want to add location info on site and set it live around that time. Check in with doctor in 2-3 weeks.

1. Homepage slider – Add darker white blur behind text so it stands out against background image. All text needs to be readable.

2. Footer – remove map and white box – replace with static image of map. Link to separate window in google maps.

Dr. Booth would like a breakdown of the charges to his account emailed to him. Please verify that he does not have any SEO or other charges that he mentioned showing up. He’s not sure if it’s with us or another company. Thanks!

HOMEPAGE EDITS

1. Referrals & Appointments green buttons in header – Remove Green Appts button, move Referrals button to right. Add Appointments link as text link in top menu next to patient forms.

2. Add space between phone number & address. Add blue phone icon before phone number, add blue address icon in front of address. Use google maps place marker symbol for address.

3. Social Icons – use circle symbols with their colors
FB = blue circle icon
G+ = Red circle icon
Twitter = light blue circle icon
Add LinkedIN icon = circle shape
Please make sure they link to his social sites, if available.

4. Slider – shorten height of sliders. Take out 1/3. Adjust images so heads are not cut off.

5. Slider text – add glow or shadow to text background. Text does not read well on mobile view because image is moved.

6. Green Treatment areas section – Remove Intake & Patient Form buttons.

7. Footer Map – Regenerate embed code so map is zoomed out to show highways. See Draft for correct map view.

8. White box on top of map – Remove logo, appt button and address. Make one column of text. Should have Office Hours, Telephone, Fax, and Email only.

9. Remove Login link in blue footer section.

10. All Inner Headers – on each of the inner pages, use each one of the different homepage sliders. Keep inner header same height.

Dr. Booth likes overall layout. Likes the colors, photos in slider- matches well with colors of site.

Edits:
1. Map at bottom of pages – increase size so shows a major highway. Move white box on left with his info inside map so it’s not overlapping footer.
2. Add “Welcome” menu link next to “Home”
3. Testimonials at bottom – wants it to randomly scroll. For example, when page is reloaded, wants a different pair of testimonials to show.

Dr. Booth will start looking through Ed. Library and make changes to content. He said the library is too general for him. Told him we can get content for him if it’s specialized. He will send us list of topics, if needed.

Told him referral form Jotform has been created and we will link to site when it is developed. We will set up SSL for form as well.

Drafts submitted via email. Had clarification with doctor but he never responded, we decided to just send him next drafts (inner pages and slides)

Thanks for the follow up.

Note focusing on the actual text/copy within the mockup, I like the layout overall. The text will need significant updating at the appropriate time but the current text can act as place holder in order to nail down look/feel which is overall pretty good from my view with what you have presented.

Specific items that will need to be addressed:

The referral form will need to be immediately present at the top right of the page for physicians and other professionals that want to quickly jump to that page of the sight to quickly enter data for a referral.

Second most important link is appointment scheduling link for patient use.

The business card-like info superimposed over the map ideally would be more integrated into the layout rather than floating freely over the map. Maybe expanding the map dimensions somewhat and providing room along the right side or left side for the information would help? It seems to hang out there and be out of place somewhat.

I will be interested to see how you set up the cycling of images in a professional way on the initial landing page as discussed previously.

Create a separate Jotform account for client and setup a “referral” page that has his referral form made in jotform. Please apply the “SSL” option when creating the form. Emailed what the form is supposed to look like.

Spoke to doctor and he confimed that he would like the site to be much more medical but also inviting to the point where patients are not afraid of coming to him. What we sent was in the scope as far as clarity and structure, but patient demographic was wrong.

He also talked about SSL separate jotform he wants. Contact information is found on his collateral.

We went off your logo colors and tried to keep the design light, and white. We used photos that hopefully invites young adults into the office. Towards the bottom of the page, we’ve added a collage of photos when a patient hovers over the area, a short description would come up. We unfortunately do not have content for women’s chiropractic care, therefore temporarily put text in that area for the moment.
