I have had some feedback on the website and need a few things addressed.

    1. Contact Us page “Advice” spelled wrong.

    2. Photos of individual physicians is extremely slow to load. I have been told that if the resolution is decreased that they will load faster. I don’t want to compromise the photos but it would be nice to have them load.

    Hi Marvin,
We are pleased the website is launched. We need on thing changed. If you click on The tab for “Contact us”.
    You see this:
Mails to the physicians are only for current patients. Do not use this e-mail if you are not a current patient, please call the office for general questions. It is against the law for the physicians to provide advise to people that are not currently being treated in this clinic.

Please eliminate the boxes below this statement and just have our patients click on the email address itself.

    —-

    Hi Marvin,

As Tina said we need to delete those boxes below that have “name, telephone, email address, comment section, and then a security word” and then put in that spot the following two sentances….1. “To send an email to your physician please click on the email address on the right side of this page”. And #2, then also add in BOLD LARGE LETTERS ” PLEASE PUT YOUR PHYSICIANS NAME IN THE SUBJECT MATTER LINE OF THE EMAIL SO IT IS DIRECTED TO YOUR DOCTOR”.

    —-

Please email Dr. Molumphy (Tmolumphy@gmail.com) and Dr. Welsh (welshjohn@sbcglobal.net) upon completion.

Thank you for changing the email when you click on "email us" BUT that also needs to have all the wording that is under the other "Contact us" regarding only current patients can use this, etc. In addition, still need to delete that area under "contact us" as described before and put "click or copy/paste the email on the right side of this page and put your doctors name in the subject line. BOTH those links "Contact us and email us" need to be lined to the same wording so as not to confuse patients.

Looks good but a couple things I found:

Under the main “Contact Us” is says patient forms coming soon BUT if you click on the quick link to “Contact Us” it brings you to a screen where you can download the forms. We need BOTH of these links to go where the patient can download and print out forms.

Second, Under the email us can you put in bold “Please put you doctors name at the beginning of your comment so will be directed to the right physician”

As requested you linked “Procedures We Perform” to the two link circles on the Home Page labeled “Post-op Spinal Management” and “Procedures” BUT you linked BOTH to that page we want completely removed which lists 30-40 procedures we do NOT perform. Just as with “quick link procedures we perform” PLEASE ALSO link these to the “Procedures” menu that then lists the cervical, thoracic, and lumbar procedures we perform and get rid of that other one all together.

    Hi Marvin,

What a difference a day makes. TX….getting there but not quite there.

Excellent on the “About Us” drop down but can you make the font bigger and darker under the “Office Policies” portion as difficult to read. Also under the “About is” drop down for each of the doctors can take out the Contact “coming soon” part at the bottom of each picture.

Great job adding the cervical transforaminal epidural drop down under “Procedures” and I have already edited it from lumbar to cervical as we discussed.

Also under “Procedures” can take out the lumbar spinal stenosis and cervical spinal stenosis on the drop downs just as you did for the radiculopathy. These are all ready under “Conditions we Treat”.

For the main page links for the circles patients click on I recommend: 1. For Post-Op Pain Mangement link that to Procedures 2. Link both the “Lumbar” AND “Cervical” circles to conditions we treat. 3. The current circle for “Procedures” links you to a BUNCH of stuff we do not do (acupuncture, iovera treatment?, splinting, stellate ganglion blocks, etc) so best to link that directly to the “Procedures” drop down and erase all those that are listed.

Than lastly, we need to get the HIPPA compliant email working as a way for patients to contact us as promised by Justin in quote.

Tina / Vanessa….feel free to chime in with any suggestions / changes that you think should be made on my suggestions above and in general.

    2016-05-09 | catherine.c
Edits done. I removed procedures dr. mentioned in her list (acupuncture, iovera treatment?, splinting, stellate ganglion blocks, etc) Let me know if there are others need to be removed.

    Hello Marvin,
I’m also hopeful that you have included the sheet that Dr Molumphy forwarded to you regarding EMG and nerve conduction studies. If you do not have it, please let me know and I will forward it to you again tomorrow. Thanks!
    Dr. Ortiz

    —-

    Hi Marvin,

It is actually adding “cervical transforaminal epidural injections” under the “Procedures Heading” and drop down to cervical. If you can add that and just put in the same language as the lumbar transforaminal epidurals then I can edit it to cervical.

The other is the drop down under “About Us”….you have the 3 doctors names BUT only brings up the entire office if you actually click on the About Us tag. No one will think to do that….need to add a drop down that says “General Office” below our three names that can be clicked on that brings up our pictures of staff and building, insurance stuff, etc.

The lumbar radoculopathy and cervical radiculopathy you added to the “Conditions We Treat” which is correct and looks good. Was not added to the “Procedure”s as per your email and should not be….

    —-

    Hi Marvin,
Thanks for the changes. I have been on the site tonight and am trying to edit Lumbar radiculopathy but it also changes Cervical. They should not be linked but completely separate. I tried to save one as Lumbar Radiculopathy but when I went into Cervical there was Lumbar…UGH!
    Thanks for the help.

    Hi Vanessa,

I do not seem to have a sheet on EMG and nerve conduction studies. I searched through my emails and I seem to only have a link (https://www.aanem.org/Patients) provided.

If you could kindly forward the sheet, that would be fantastic.

    Hi Dr. Molumphy,

I will see what is going on. It seems as if you are editing the same page that is linked twice. I will look into the matter and try to adjust it the way you would want.

    Hi Dr. Welsh,

Thank you for your clarification. I will respond back shortly with a itemized list of changes so we can be on the same page.

Hi Doctors! The "Thoracic Transforaminal Epidural Injections" content was copied onto the "Cervical Transforaminal Epidural Injections" I have added "Cervical Transforaminal Epidural Injections" and "Thoracic Transforaminal Epidural Injections" under their respective navigation menus. The EMG Form was added as a downloadable link on the "Electrodiagnostic Testing (EMG/NCS)" Page (http://1cc.ef2.myftpupload.com/education/electrodiagnostic-testing-emgncs/) "Lumbar Radiculopathy" Page content was copied onto the "Cervical Radiculopathy" Page "Lumbar Radiculopathy" and "Cervical Radiculopathy" were removed from the Procedures menu. "Lumbar Radiculopathy" and "Cervical Radiculopathy" were added and linked appropriately on the "Conditions We Treat" page. (http://1cc.ef2.myftpupload.com/conditions-we-treat/) Quick Links has been updated by taking out the Insurance content from the Legal Notices page and putting Insurance content on a separate Insurance Page that is also added on the Quick links. The Main Menu has been reorganized as shown below: Home About Us (Not a Page anymore) Dr. Vanessa Ortiz Dr. John Welsh Dr. Tina Molumphy About the Office Staff About Physiatry Office Policies Conditions We Treat Testimonials Procedures Lumbar Lumbar Spinal Stenosis (Need content) Electrodiagnostic Testing (EMG/NCS) Transforaminal Epidural Injections Media Branch Blocks Radiofrequency Ablation of the Lumbar Facet Joints Thoracic Electrodiagnostic Testing (EMG/NCS) Thoracic Medial Branch Blocks Radiofrequency Ablation of the Thoracic Facet Joints Thoracic Transforaminal and Interlaminar Epidural Injections Cervical Cervical Spinal Stenosis (Need content) Electrodiagnostic Testing (EMG/NCS) Medial Branch Block Radiofrequency Ablation of the Cervical Facet Joints Cervical Transforaminal Epidural Injections Cervical Translaminar Epidural Injections Cervical Epidural Steroid Injection Contact Us Email Patient Forms Brochures for Patients Additionally, the frontpage has the Lumbar and Cervical icons leading to a blank page -- let me know what you would like to do for this page -- if you want to add specific content, put a list of articles (like the conditions we treat page) or link it to a current page with content already. Please let me know if I missed anything!
    2016-05-03 | marvin.a
Hi Marvin, There are two drop down bars under "Contact Us" that belong under the "About Us". 1. About the office staff 2. Meet the staff (change to "About Physiatry") this is the picture with the three docs and how our relationship was built/ definition of Physiatry. I would greatly prefer this page be the taken to when you click on the home page "read more". 3. Can the page titled about us (not the bar at the top), be changed to "Office Policies" and another drop down. So here is a summary: About Us Dr. O Dr. W Dr. M About the office staff About Physiatry Office Policies
    2016-05-03 | marvin.a
One other I noticed Marvin is under "Quick Links" one is labeled "legal notices" which is good for the first half of the page it takes you too...but need to have a separate "Quick Link" that says "Insurance Information" and then put the insurance info under that link. Currently BOTH the legal notices and insurances is under the "Legal Notice" quick link and no one would figure to click on that to see what insurances we take. Also still need to add "Cervical transforaminal epidural injections" under the drop down for cervical procedures we perform. I can edit it to our liking (so you can just put the lumbar transforaminal epidural definition there also if you do not have one specific for cervical transforaminal epidural injections and then I will edit it) but I do not know how to create the actual drop down with that heading.

Tina, great job on the website. Definitely coming together. Was looking at it today and a few changes you can make Marvin are the following:

Under contact us: “general” is not spelled correctly [done]

Believe we should add: Lumbar sciatica (radiculopathy), Cervical radiculopathy (pinched nerve in neck), and cervical and lumbar spinal stenosis under the part headed “Conditions we treat” Right now there is just degenerative disc disease (which you can leave)

Under treatment should take out “Herbs” as we do none of that

Take out the ability of patients to add new testomonials [done]

Thank you for all the hard work Tina and your help Marvin in making this website look its best. Love the pictures!

    Notes | 2016-04-20 | Marvin A. | #49409 |

Just letting you know that your patient form packet (12 pages) is on your Contact Us page.

We do offer a service to convert your packet into a digital copy. Patients will be able to download it and fill it out on their computer. This way they can send the forms digitally without having to print out the form. Even if they do decide to print out the form physically, it will look a lot nicer without the streaks and margins. While we normally charge $199 per two pages, I have been approved to offer $299 for 12 pages (your entire packet). Please let me know if this interests you!

“Dr. Ortiz has been an absolutely wonderful pain management physician. She has always been there for my pain management needs. I would highly recommend her to everyone in need of pain management—she is very empathetic.”

    James O.

“Dr. Ortiz, her partners and staff are caring professionals and work in concert to make the patient experience exceptional. Dr. Ortiz listens to patient input, concerns and questions and combines that with her knowledge of cutting edge medical treatment, testing and pharmacological options to ensure a proper diagnosis and treatment. The result is a successful treatment plan that the patient understands and feels part of. Pain management is a journey. Dr. Ortiz guides you and continues to look at all options to ensure your best results along the way.”

    Ron D.

“KUDOS TO DR. VANESSA ORTIZ AND DR. JOHN WELSH

As a Director of Nursing, a patient advocate, and a wellness coach, one of my responsibilities is to ensure that residents under my care will not only avail the quality of care they deserve, but also to be respected with their choices and options for their entire wellbeing. When choosing a healthcare professional among the many options out there, it is our priority to choose a professional who values a patient as a whole person, not just a part of a job that needs to be accomplished. We also look for a doctor who gives a gift of time and explains their plan of care and treatments. I had the opportunity to meet Dr. Ortiz and Dr. John Welsh when one of my patients was referred to their office. Our wonderful experience started with a cheerful and efficient staff. Then, meeting all our goals and expectations with flying colors, after our first appointment made us feel that we are truly blessed to be in their care. After all the treatment was done, we put Dr. Ortiz and Dr. Welsh on our TOP Doctors’ list. Since then, I brought several patients and never had any moment that their quality of care has lessened. In fact, it gets better all the time. My profound gratitude to all of you for being there! Thank you.”

Grace P., BSN RN PN CEC

Dr. Welsh is far and away the best doctor I have ever met and nothing I can say really does justice to the man: He is hands-down the best medical doctor I have seen. He is extremely intelligent, a compassionate and caring person, and a truly outstanding doctor. Being in Law Enforcement for over 25 years now, I was referred to him 15 years ago after having back surgery for an on duty accident. I count myself truly lucky to have found him.At every visit, he makes you feel comfortable and relaxed, and truly cares for your well being. He takes the time needed to fully understand your concerns and explains what your options for treatment are. He helps you make educated decisions on your care. nFrequently, we spend time just chatting and catching up on any number of things. He is interested and is interesting; a good combination, trust me on this: you will like the man. The office staff is a projection of Dr. Welsh. They are all very attentive to your needs, personal and truly listen. They are courteous, very friendly and welcoming whether in person or on the phone.

    Bruce Y.

Dr. Welsh has been my pain management specialist for 12 years. He is a superb doctor who can best be described as a patient advocate who goes out of his way to help his patients. He also has a great bedside manner. He has had to fight hard with my insurance company to get them to provide me with the proper medical coverage that I needed and he has done this willingly. Before treating with Dr. Welsh two previous doctors had incorrectly diagnosed my back problems. When Dr. Welsh was recommended to me he agreed to look at all my medical records free of charge before we had even met. When I met Dr. Welsh for the first time he correctly diagnosed my problem within the first 10 minutes and explained that I had a pinched nerve in my lower back. His office is a one stop shop. He and the other doctors in his office perform the necessary electro-diagnostic tests to determine the exact nature and location of your pain and Dr. Welsh himself does the lumbar and cervical epidurals. His efforts have kept me off the operating table for 14 years and has successfully managed my pain level. I am grateful to Dr. Welsh and his staff for their knowledge and for the professional treatment I have received from them. I highly recommend Dr. Welsh.

    Bill B.

    Client Complaint
    Complaint Notes | 2016-04-04 | Justin | #48216 |

Client left me a voicemail asking to speak with an account manager or a higher position in the company – called her back – she was concerned about her site – why is it taking so long (content wise) – certain things that were supplied at the beginning have not been placed yet – went over the whole design process with her so we are on the same page – the design comes first then content – she gets up to 5 (custom) – wants to see educational library – asked how long marvin has been working and if he is new – assured that he is competent enough and one of my favorites – told her I will address her concerns and take care of things – she was very happy to hear and is awaiting educational library and marvins email

Hi Dr. Molumphy, I had just talked to Justin and he said you voiced some concerns about your website. I apologize for anything that I have been doing to make you feel this way. I would like to reassure you that I am doing what i can to make sure your site can be ready to go live as soon as possible. You had brought up (http://www.remedydocs.com/index.html) during one of our phone calls as an example of the type of services and procedures you offer. For the most part, that is the list we had to go on. If there is anything currently on your site that you do not offer, please let me know just like you did this morning. Your speciality has very specific subject matter, but I feel it closely relates to physical therapy. I would like to suggest that you take a look if there is anything within our library for the related specialities I will link below so that I can be more accurate in preparing the content for your site. Physical Therapy I've also included somewhat related fields below. Let me know if we can use any of the content. Sports Medicine Chiropractic As for the PDFs you gave me at the start, they are mainly anatomy imagery. If you can give me any direction where you would want them, that would be helpful. We can make them links on a current page, I would just need to know which pages they should be on or we can just make an overall library page as well. If you have any clarification or further concerns, please let me know!

Los Altos Spine and Sports is NOT a legally established partnership or corporation. This internet domain name is being used so as to have a central site where patients can go for medical information, common questions regarding our office, and HIPAA compliant email correspondence between doctors and their established patients.

This type of association of specialists (each having their own independent medical practice but sharing medical office space) is frequently done for multiple beneficial reasons, including continued medical educational growth as well as cross coverage of patient in emergency situations when a patient’s treating physician is not available.

In addition, sharing in the continued rising cost of maintaining an independent medical practice is greatly enhanced when several physicians split the continued elevating overhead of running a well-staffed and efficient medical practice.

    Accepted Insurances:
Each physician is a contracted/in-network provider for Medicare and most all major PPO insurances including Blue Cross of CA, United Healthcare, Cigna, Aetna and HealthNet.

None of the physicians are contracted with any HMO insurances.

In addition, each of the physicians is not contracted with any of the Covered California Programs run through Blue Cross, Blue Shield, HealthNet, Cigna or Aetna (this also includes any Pathway or Pathway X programs.) Such patients are still seen at our office, but will use their out of network benefits which require payment in full, at time of service. As a courtesy to these patients, a claim can be submitted to your insurance company upon your request.

    Quick Changes
    Design | 2016-03-22 | Marvin A. | #47330 |

    1. Instead of Additional treatments, lets change that to Spinal Procedures
    2. At the bottom of the page under my picture, I have 17 years experience, Welsh has 18 and Ortiz 19.
    3. Make and appt. Does that generate an email to my front staff? If they get too many emails we can take that down right?

    Thanks,

    Hi Dr. Molumphy,

For #3, we can decide what email the form generates to — you are allowed up to 2 I believe. If there is too much spam, we can take it down.

We do have our general company emails attached to that so we occasionally monitor the forms in case of spam. Thus far there hasn’t been any issues with any of our clients. Worst comes to worst we can try to put a captcha before taking it down.

    Hi Sachit,

    This client is a Physiatrist.

Make sure there it is clear there are 3 independent contractors instead of a “Group”. Dr. Tina Molumphy, Dr. John Welsh. Dr. Vanessa Ortiz

Would like the site to be focused on active and spine imagery. The deal more with spine than with sports related injuries. They are around the silicon valley area, so try to be contemporary.

They do not have a logo. No colors are chosen, it is up to you.

They would like to go with the new trend of the vertical scrolling sectioned hompage.

Try to target middle-aged professionals that work at tech startups.

(http://healthbridgechiro.com/welcome-to-healthbridge-chiropractic/) Dr likes this site, but please make it more contemporary and less busy. Just note that it is a very tech/scientific feeling.

    Hello Dr. Molumphy,

    Welcome to Optimized360!

My name is Marvin and I will be the designer working with you every step of the way, starting from concept to going live! I had a quick meeting with Justin this afternoon to discuss your account. We have purchased losaltosspineandsports.com on your behalf. Justin had let me know that you are available tomorrow from 830-9AM. I get into the office at 830AM PST, is it possible to call you around 835-840 AM PST. Our call shouldn’t take more than 15 minutes.

Please take a few minutes to fill out our design questionnaire below. This should give me a general idea of what you would like to see in your design.

*Please Login to Control Panel (www.360controlpanel.com)
*(Feel free to contact me if you need any assistance logging in)
    Username: Tmolumphy@comcast.net
    Password: 360_tmolumphy

You can find the Design Questionnaire in the welcome box.

Additionally, if you have any examples of sites you liked and were visually appealing to you (this doesn’t have to be strictly from our gallery) let me know. If you also have a logo or any office pictures so that I can gain a better sense of the design style we should approach to tie everything together, that would be great.

I look forward to working on this design with you.

Thanks for the quick reply. I know we spoke about a telephone follow up on Thursday but I’ll be in Yellowstone!

    1. Is the HIPPA compliant email for each of us 3 practitioners? Or one email? How does that work?
    2. Would you be coming up to the Bay Area to meet with us at any point?

    Dr. Molumphy,

Thank you for your updated response.
I understand that you have quotes from other companies and I will be able to MATCH & BEAT any proposal by 10% if they offer anything similar to what we are offering you… that is how confident I am that we have a superior product.

Our company also offers a 30 Day Money Back Guarantee, so you can essentially see your design draft today — no risk, no obligations, no contract.

    The monthly maintenance fee.
We are an extremely small practice of three physicians with 4 staff employees. We have a very narrow field of medicine (spinal pain management) and a dozen or so procedures that don’t ever change! I can’t imagine we would be requiring many alterations to the website after the first couple of months. $99/month is triple what another company offered. Is this negotiable? Or possibly a fee for service?

Dr. Molumphy, I am very familiar with our competitors and I assure you that those companies with a very low monthly fee have hidden fees involved. They are a business and they need money to survive so they will charge you for every little thing that you would like to do and over time it would add up.

Our monthly fee includes: hosting, maintenance, technical support to the website, unlimited updates (pictures/content), access to your very own control panel (check emails, submitted support tickets, see statistics of your site), and lastly… your requested HIPAA Email Account.

If it would help, I will personally waive the first 4 months off your monthly fee ($400 Savings).

Links to articles? I often read articles in journals, newspapers, magazines, etc. is there a way for me to upload them directly? Or would we have to go through you?

Great question! Yes, if you would need our website specialists help, we can definitely upload it and credit the source for you.

    OR

If it is night time, you can’t sleep, you have the capability of going into the website, accessing your files right then and there to make the changes instantly!

Facebook, Twitter, etc. we don’t have accounts for work with these. Should we? Would you help us set these up if so?

I’ve already personally included for the social branding to be set up for your practice. (Reg. $999)
It has been added to your package at NO ADDITIONAL COST.

Please see the updated proposal in which I have authorized to accommodate you better.
Your website is a long term investment… I assure you that if you save that extra $1,000 to build a template site… you will not see a great return over time as oppose to our Design from Scratch Website & Custom Unique Content in which we will provide for you.

As addressed before as well, if you were to go with a template based company that offers generic content — it would have lesser value in Google’s eyes when it comes to your ranking, as opposed to our Unique Designs from Scratch with Custom Rich Content.

This not only effects your image, your conversion, but your exposure as well if you work with these template based companies that charge 2-3 dollars less.
Our company will be the best investment for your practice and I know that when you look back 10 years from now, you will be happy you made the right choice in working with Optimized360.

    Thank you,

    Hi Justin,
Thank you for putting this together for my practice. I have reviewed this information/quote and the sample websites you have sent. I am pleased with most of what I have reviewed. I do have other quotes from other companies and thus have some questions.
    1. The monthly maintenance fee

    2. Links to articles? I often read articles in journals, newspapers, magazines, etc. is there a way for me to upload them directly? Or would we have to go through you?

    3. Facebook, Twitter, etc. we don’t have accounts for work with these. Should we? Would you help us set these up if so?

    Sincerely,
    Tina Molumphy
