I would like an opportunity to speak with you as the quality director for one of the largest clinic systems in the state of Iowa, The Iowa Clinic. The conversation would be how you may value quality reports for your specialists and your clinic system. I’m in the process of launching a service for specialists who care-for/treat patients in an inpatient or outpatient setting. Reports can include a quarterly breakdown of:
· gross billable charges
· percent of market share
· relative level of activity
· payor mix, commercial insurance, HMO, Medicare, Medicaid, other
· 3M severity adjustment and indexing for length of stays and average charges compared to their peers and the hospital average
· mortality rate, raw data and indexed
· complication rate, raw data and indexed
· severity adjusted case mix index
· percentage breakdown of their caseload in a minor, moderate, major, and extreme severity category
· a breakdown in their utilization of resources; routine services, ICU, pharmacology, implants, lab, radiation, OR, respiratory therapy, physical therapy, ER, cardiac, other
· breakdown of patients treated under specific complication diagnosis and comorbidities
· breakdown of the origin of their patients by zip code, number treated, their APR DRG’s, the specialities provides and descriptions, length of stays and charges by hospital in which they were treated
· additional reporting can include their rank within the area researched as compared to their peers including aspects of volume percent of facility totals and percentage of the total database being reported

Would having access to information like this be of value? Considerations may be:
· the future probability of a pay for performance program by the government (Depending on level of interest, the Medicare Medicaid data set will be the first one I access and I’m using the same program the government uses to analyze outcome data – the 3M APR DRG, the gold standard for medical data. With adequate demand, I will acquire the all-payer-database.)
· your ability to accurately rank your staff and compare their outcomes to other specialists in their primary hospital, across town, the state, or the country
· providing a tool consideration in payroll negotiation
· providing an ability for recognition
· Providing of ability for marketing! When you have specialists that rank head and shoulders above their peers tell your clients.
· Establish a basis for peer training and improvement. Have your best in class physicians coach colleagues!

Advocacy systems are using this data today to navigate patients to higher-quality outcomes.

Tony called, requested a refund, he went on to explain the project never took off and we did not do any work as he was going through legal issues and kept holding off. I explained we sent him some logo drafts and the best we could do was refund a portion $1,898 ($2,698 - $800) as we were continually trying to contact him to finish the project. I also explained he would not get a better deal than the one he received and urged to follow this whole thing through. and complete the project. He mentioned he will reach out again next week.
