Background Info: This client has cancelled with us, however I would like the design to remain active. We have to remove change all doctors names, contact information to dummy text.

Instructions:

1. Please change the doctor’s name to Dr. John Smith and use our company contact information.
2. Replace office photo with an office photo from bigstock. We have a few downloaded already too.
3. Remove all pages and load our oral surgery XML into it. Please replace brackets with our info as well.
4. Let Belinda know when completed.

Client filed a dispute for the 2nd 50% initial charge. I had spoke to client to try to get the issue reconcile. after 30 minutes, client and I had agreed to part ways and that i will speak to owner to get the 2nd 50% refunded.

2 hours after the phone call, maryana had filed another dispute which this time is for the initial 50%. I had called her back but office was closed for labor day. I sent her an email letting her know about what I had to do to get the 1st dispute reimbursed but now this is file which was uncool going against the agreement. I spoke to dr f and dr f stated we will send it to collections if she doesn’t remove the 1st 50 dispute. Pending her response.

Belinda, please stop all process for their account.

Chan, please stop charges. We will also be refunding $1,750 per the credit card dispute. Dr Fahimi ok’d to process/resolve this with the cc company. Let me know when done.

Client has been unresponsive, I have sent a few emails to follow up and they have been read/sidekicked. Last I spoke with client, she was unhappy with the slow process of everything and has requested to keep point of contact to a minimum. I will give her a call early next week to follow up.

I am not contacting her as she would like to keep points of contact at a minimum. I don’t see a follow up since June 29th and that was from her. Please be sure to keep in contact with your clients especially if they are not live with their website.

Remove referring doctors page. Make the “Referring Doctor” menu button go to the referring doctor pdf.

Have a drop down menu under “Referring Doctor” include the following items: Referral Form, Link of Interests, Study Club, Oral Surgery News.

Use original site content for procedures, not the new content.

Add “Surgical Instructions” into main-menu. (get info from original site)

Change Patient Forms  to “Patient/Consent Forms” under the “Forms” link.

Can you please log onto FTP and add the following lines to the end of the robots.txt file:

Important for you to FLUSH CACHE in wordpress after the changes have been made.

Patient Form, checkboxes not working.

Replace logo with the one on website.

Forms: Botox Consent stays the same, the remaining consent needs to be checked on and updated to what emailed.

Videos, please add individual on to each procedure page. Update Dr’s bio with picture labeled “bio.jpg”

Have been trying to contact Maryana to follow up with the remaining of the products. She’s received messages and mention she will contact us.

Website has fillable forms done by PBHS that we cannot transfer over. I have sent her our generic forms that we may put on the site, but if she would like them to be fillable, the cost is $250.

She’s mentioned that she was promised to get the fillable forms but her forms are about 10 pages long and told her we have our own version and can supply that to her only.

I’ve updated the draft based on your feedback but have just one question that I listed below as well.

1. Please make bigger space in Calabasas Oral Surgery sign between the words

2. Make the view picture narrow – I cut about 30% of the height off.

3. Did not like the new picture ( color is off) — I placed the photo exactly how the original is but just cropped.

4. Please change office picture to reflect waiting area

5. Move tweeter, facebook icons to the top next to the phone #

You gave me two colors to use #3bboff / #2bb8ff, which showed up as a sky blue; therefore I created another version of the design in the blue colors. Please take a look and let me know which color set you’d like to go for. Thank you!
