I’d like to keep the two images at the top of the page, directly followed by the YouTube video. I’m working on a new paragraph of text to place underneath the YouTube video to welcome page visitors to this page of my website.

After my new paragraph (which I will send separately), I’d like to start with the “Snoring isn’t sexy” text all the way through to the “Other Therapy Attempts”, then I’d like The Epworth Sleepiness Scale to be the last thing on the page.

Lastly, when someone submits their score on The Epworth Sleepiness Scale, the contact form pops-up. Can you please change the text from “Make an Appointment Today!” to instead read as “Get More Information Today!”.

I’d like the Name to be a required field, along with email and comments.

Can we put the side by side image of patient wearing a cpap, vs wearing an appliance(nocpap) next to CPAP sucks at top of the page I would also like to have a contact us form for patients to fill out towards the top of the page, so patients do not have to scroll all the way to the bottom. Third I want an online form the epworth sleepiness scale, where it automatically calculates their score based on their responses. See attached file. Then it prompts them to contact us.

What is my google analytics log in and password? Or could you change the setting to Enable Demographics and Interests reports.
